Efficacy and safety of GnRH-a combined with ultrasound-guided percutaneous microwave ablation in the treatment of uterine fibroids.

Evaluation of a phytogenic blend supplementation on reproductive performance, antioxidant status, and immune function in broiler breeder roosters.

Rooster fertility declines with aging, marked by reduced testicular weight, testosterone, semen quality, and antioxidant capacity, leading to lower hatching egg production and economic loss. This study evaluated the physiological effects and optimal inclusion rate of LibidoMax, a phytogenic supplement, on reproductive hormones, semen quality, antioxidant status, immune function, and metabolic biomarkers in aged broiler breeder roosters, using broken-line regression modeling. Twenty roosters (42 weeks old) were randomly assigned to four treatments (0, 100, 200, or 300 mL LibidoMax per 100 kg feed) for 70 days, including 14 days of acclimation. LibidoMax significantly enhanced serum levels of gonadotropin-releasing hormone (GnRH), luteinizing hormone (LH), follicle-stimulating hormone (FSH), and testosterone (P < 0.001). Broken-line regressions identified optimal hormonal plateaus between 182-190 mL/100 kg feed. Semen volume, sperm motility, viability, and concentration improved linearly, with breakpoints around 200 mL/100 kg feed, while sperm abnormality was minimized at ∼179 mL/100 kg feed. Hematological parameters improved with supplementation. Hemoglobin (HGB) and hematocrit (HCT) reached breakpoints at 176.5 and 179.6 mL/100 kg feed, respectively. Red and white blood cell counts (RBC and WBC) plateaued at 178.7-184.1 mL/100 kg feed, indicating enhanced oxygen transport and immune activation. Antioxidant enzymes, including total antioxidant capacity (T-AOC), glutathione peroxidase (GPx), and superoxide dismutase (SOD), peaked at ∼200 mL/100 kg feed, while malondialdehyde (MDA), a lipid peroxidation marker, was minimized at 100-110 mL/100 kg feed, reflecting early saturation of oxidative stress reduction. Lipid profiles were favorably modulated: triglycerides (TG), low-density lipoprotein (LDL), very low-density lipoprotein (VLDL), and total cholesterol decreased (breakpoints: 169-178 mL/100 kg feed), whereas high-density lipoprotein (HDL) peaked at 182 mL/100 kg feed. Liver enzymes, alanine transaminase (ALT), aspartate transaminase (AST), and alkaline phosphatase (ALP), were reduced at 170-180 mL/100 kg feed, indicating improved hepatic function. Principal Component Analysis (PCA) confirmed multivariate separation among treatment groups, with LibidoMax positively associated with reproductive, antioxidant, and immune enhancements and negatively with hepatic stress and lipid markers. In conclusion, 180-200 mL/100 kg feed represents the optimal inclusion range of LibidoMax to improve reproductive efficiency, immunity, and systemic health in aged breeder roosters.

Technical report on preservation of dromedary camel embryos at 4°C for up to 72 h in catalase-supplemented media.

Intrauterine infusion of oil-based medium in women with recurrent implantation failure before frozen embryo transfer: a randomized clinical trial.

Oil-based contrast medium in hysterosalpingography has been recommended as an approach to improve fertility in recent years. However, this approach has rarely been applied to patients with unknown recurrent implantation failure (RIF). This randomized clinical trial aimed to assess the effects of intrauterine oil-based contrast infusion on frozen embryo transfer (FET) prognosis and pregnancy outcomes among patients with unknown RIF. This multicenter prospective randomized clinical trial was conducted between November 2019 and December 2022. A total of 224 patients with unknown RIF were randomized to the intrauterine oil-based medium infusion group or the control group at a 1:1 ratio. The oil-based contrast group was intrauterine infused with ethiodized poppy seed oil at a dose of 5 ml before FET, and the control group underwent only a standard FET. The primary outcome was the live birth rate (LBR). The secondary outcomes were implantation rate, clinical pregnancy rate (CPR), ongoing pregnancy rate (OPR), and early miscarriage rate. One hundred patients completed intrauterine oil-based medium infusion followed by FET within 120 days. The intention-to-treat analysis demonstrated balanced baseline characteristics between groups. The oil-based medium group exhibited significantly higher CPR (56% vs. 37.4%, rate ratio (RR) 1.498, 95% CI 1.109-2.023, P = 0.007) and implantation rates (median 50% [interquartile range, IQR 0-100%] vs. median 50% [IQR 0-50%], P = 0.010) compared to controls. However, no statistically significant differences were observed in LBR (39% vs. 27.1%, RR 1.439, 95% CI 0.969-2.138, P = 0.069) or OPR (40% vs. 27.1%, RR 1.476, 95% CI 0.997-2.168, P = 0.049), with the latter approaching borderline significance. Subgroup analyses revealed enhanced CPR specifically in RIF patients aged 35-39 years (P = 0.041) and with gonadotropin-releasing hormone agonist (GnRHa) pretreatment hormone replacement therapy (HRT) (GnRHa-HRT) endometrial preparation protocol (P = 0.008). However, no disparities were observed in the LBR and OPR. Within-group comparison of oil-based medium infusion protocols showed that GnRHa-HRT significantly improved reproductive outcomes versus HRT protocols, including LBR (52.2% vs. 26.1%, RR 1.894, 95% CI 1.035-3.466, P = 0.023), CPR (71.7% vs.39.1%, RR 1.983, 95% CI 1.198-3.283, P = 0.002), OPR(P = 0.047), and implantation rate(P = 0.021), which might be attributed to the thickening of the endometrium (P = 0.027). Infusion with an oil-based medium did not increase the LBR in terms of FET prognosis among patients with unknown RIF. However, compared with HRT, an oil-based contrast agent combined with a GnRHa-HRT FET increased the endometrial thickness and the LBR. These findings suggest synergistic effects of dual interventions, proposing a potential personalized strategy for refractory RIF cases. Further large-scale RCTs and deeper mechanism studies are warranted to validate clinical applicability. The role of pre-operative gonadotrophin-releasing hormone agonists (GnRHa) on pain, bowel and bladder symptoms in rectovaginal/colorectal endometriosis surgery: a multicenter cohort study.

The efficacy of medical and surgical treatment of endometriosis-associated pain is a source of ongoing controversy. There is a lack of evidence about gonadotropin-releasing hormone agonists (GnRHa) use on long-term pain control, bladder and bowel symptoms for patients having surgery for deep rectovaginal/colorectal endometriosis. To assess the effect of preoperative GnRHa (pre-GnRHa) use on pain, bowel and bladder symptoms for patients undergoing surgery for deep rectovaginal/colorectal endometriosis. The study evaluated data from the British Society for Gynaecological Endoscopy database, a large international multicentre prospective cohort of patients who underwent deep rectovaginal/colorectal endometriosis surgery between 2009-2021. We included 9433 patients from 101 accredited endometriosis centres. Multivariable logistic regression analysis was used to evaluate the association between pre-GnRHa use and postoperative pain, bowel and bladder symptoms at different time points, controlling for confounders like patient age, body mass index, smoking status, and hysterectomy. Rate of cyclical and non-cyclical pelvic and menstrual pain, bowel and bladder symptoms. The mean age of the patients was 36 years (18-55). Pre-GnRHa use was associated with significant postoperative improvement in premenstrual pain [odds ratio (OR): 0.30, 95% confidence interval (CI): -0.57 - -0.034, <i>P</i>=0.02*], menstrual pain (OR: 0.41/10, 95% CI: -0.7 - -0.13, <i>P</i>=&lt;0.001*), non-cyclical pain (OR: 0.27/10, 95% CI: -0.5 - -0.04, <i>P</i>=0.021*) and lower backache (OR: 0.30, 95% CI: -0.532 - -0.087, <i>P</i>=0.006*) up to 12 months postoperatively. Moreover, bladder pain was significantly reduced in the pre-GnRHa group at 12 months (OR: 0.24, 95% CI: -0.451 - -0.039, <i>P</i>=0.01*). Significant improvements were observed in bowel symptoms including frequent bowel movements (OR: 0.10, 95% CI: -0.194 - -0.012, P=0.02*), incomplete emptying sensation (OR: 0.10, 95% CI: -0.196 - -0.023, <i>P</i>=0.01*), cyclical dyschezia (OR: 0.43, 95% CI: -0.724 - -0.142, <i>P</i>=0.003*) and non-cyclical dyschezia (OR: 0.28, 95% CI: -0.504 - -0.075, <i>P</i>=0.008*) up to 12 months. Pre-GnRHa use is associated with a significant reduction in postoperative menstrual pain and non-menstrual pain as well as improved bowel and bladder symptoms lasting up to two years. It is also valuable to improve the quality of life for patients undergoing surgery for deep rectovaginal/colorectal endometriosis. This is the largest prospective international study evaluating pre-GnRHa use in deep rectovaginal/colorectal endometriosis surgery. It provides evidence supporting the role of pre-GnRHa as an adjuvant to surgical treatment, to reduce postoperative pain and improve bowel and bladder function.

Association of combined exposure to air pollutants during ovarian stimulation, follicular fluid metabolites, and IVF/ICSI outcomes: A prospective cohort study.
